Indication
Used for the treatment of severe recalcitrant nodular acne that has not responded to conventional therapy including systemic antibiotics. Isotretinoin is a retinoid (vitamin A derivative) that reduces sebum production, normalises keratinisation, and has anti-inflammatory effects.
Ingredients

Direction
Adults and adolescents: Dose is individualized, typically 0.5–1 mg/kg/day in divided doses. The cumulative dose to achieve remission is usually 120–150 mg/kg. Take with food (high-fat meal improves absorption). Do not crush or chew capsules. Course typically lasts 16–24 weeks.
Side effects
- Very common: dry skin
- chapped lips
- dry eyes
- nasal dryness
- nosebleeds.
- Very common: elevated triglycerides and cholesterol.
- Common: musculoskeletal pain
- arthralgia
- myalgia.
- Common: headache
- fatigue.
- Common: transient hair thinning.
- Uncommon: depression
- mood changes.
- Rare: intracranial hypertension (headache
- visual disturbances)
- pancreatitis.
- Very rare: severe mucocutaneous reactions (Stevens
- Johnson syndrome).
- Very rare: inflammatory bowel disease.
Precautions
- PREGNANCY PREVENTION PROGRAMME: Isotretinoin is teratogenic — causes severe birth defects. Women of childbearing potential must use two forms of effective contraception for at least 1 month before
- during
- and for 1 month after treatment.
- Contraindicated in pregnancy and breastfeeding — ABSOLUTELY.
- Contraindicated with tetracyclines — increased risk of intracranial hypertension.
- Do not donate blood during treatment and for at least 1 month after stopping.
- Monitor liver function tests
- blood lipids
- and blood count before and during therapy.
- May cause depression
- mood changes
- and suicidal ideation — monitor closely.
- Do not use vitamin A supplements concurrently.
- Avoid waxing
- dermabrasion
- or laser treatment during and for 6 months after therapy (risk of scarring).
- Avoid prolonged sun exposure and use sunscreen.
- Keep out of reach of children.
