Amarel 2 mg

Indication

Amarel (Glimepiride 2 mg) is a third-generation sulfonylurea oral antidiabetic used to control blood glucose levels in adults with type 2 diabetes mellitus, as an adjunct to diet and exercise. It stimulates insulin secretion from pancreatic beta cells.

Direction

Adults: The usual starting dose is 1 mg once daily with breakfast or the first main meal. Dose may be increased in 1 mg increments at 1–2 week intervals based on blood glucose response, up to a maximum of 6 mg/day. Take immediately before or with the first main meal of the day.

Side effects

• Hypoglycaemia (low blood sugar) — common and can be serious.
• Nausea
• vomiting
• diarrhoea
• abdominal pain — uncommon.
• Weight gain — common.
• Elevated liver enzymes
• jaundice — rare.
• Blood dyscrasias (agranulocytosis
• aplastic anaemia) — very rare.
• Photosensitivity
• skin reactions — rare.

Precautions

• Do not use in type 1 diabetes
• diabetic ketoacidosis
• or if allergic to glimepiride or other sulfonamides.
• Hypoglycaemia is the most serious risk — ensure adequate food intake and do not skip meals.
• Dose reduction required in renal or hepatic impairment.
• Caution in elderly patients (increased hypoglycaemia risk).
• Monitor blood glucose regularly.
• Alcohol can potentiate or antagonise the hypoglycaemic effect.
• Interactions with NSAIDs
• other antidiabetics
• ACE inhibitors
• fluconazole (enhance hypoglycaemia)
• and with corticosteroids
• thyroid hormones (reduce effect).
• Consult a doctor before use in pregnancy or breastfeeding (insulin preferred).
• Keep out of reach of children.
