Indication
Diabetmin (metformin 500 mg) is a biguanide antidiabetic used as first-line pharmacotherapy for type 2 diabetes mellitus, particularly in overweight patients, when diet and exercise alone are insufficient to achieve glycaemic control. Also used off-label in polycystic ovary syndrome (PCOS).
Ingredients

Direction
Adults: Initially 500 mg 2–3 times daily with or after meals; increase gradually based on blood glucose response. Usual maintenance dose: 1,500–2,000 mg/day in 2–3 divided doses. Maximum: 3,000 mg/day. Take with meals to reduce gastrointestinal side effects.
Side effects
- Nausea
- vomiting
- diarrhoea
- and abdominal discomfort (most common
- especially at initiation).
- Metallic taste.
- Vitamin B12 deficiency with long
- term use.
- Rare but serious: lactic acidosis (particularly in predisposed patients).
Precautions
- Contraindicated in renal impairment (eGFR < 30 mL/min/1.73 m²) — risk of lactic acidosis.
- Suspend before iodinated contrast media procedures and restart 48 hours after if renal function is stable.
- Contraindicated in hepatic failure
- excessive alcohol intake
- and conditions predisposing to hypoxia.
- Temporarily discontinue before major surgery.
- Monitor renal function at least annually.
- Keep out of reach of children.
