Indication

Management of type 2 diabetes mellitus in adults as an adjunct to diet and exercise when both sitagliptin and metformin are appropriate. Sitagliptin is a DPP-4 inhibitor that increases incretin levels; metformin reduces hepatic glucose production and improves insulin sensitivity.

Direction

Adults: The recommended dose is one tablet of Sitagliptin 50 mg + Metformin 500 mg twice daily with meals. The dose should be individualized based on glycaemic response and tolerability. Take with food.

  • Headache
  • Hypoglycaemia when combined with insulin or sulfonylurea
  • Pancreatitis (rare)
  • Lactic acidosis (rare but serious)
Precautions
  • Contraindicated in patients with eGFR < 30 mL/min/1.73 m² (severe renal impairment).
  • Contraindicated in metabolic acidosis including diabetic ketoacidosis.
  • Withhold before and 48 hours after iodinated contrast media administration.
  • Risk of lactic acidosis with metformin — caution in conditions predisposing to hypoxia.
  • May cause pancreatitis — discontinue if symptoms develop.
  • Sitagliptin may cause joint pain (arthralgia) — discontinue if severe.
  • Use with caution in elderly patients due to potential renal impairment.
  • Keep out of reach of children.
