Indication

Ingredients

• Empagliflozin 12.5 mg
• Metformin hydrochloride 1000 mg

Direction

Take exactly as prescribed, with meals, and maintain adequate hydration.

• Kidney function should be assessed. Seek urgent care for symptoms of ketoacidosis or lactic acidosis. Discuss fasting
