Indication
Treviamet (Sitagliptin 50 mg + Metformin HCl 1000 mg) is a fixed-dose combination antidiabetic used as adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes mellitus. Sitagliptin is a DPP-4 inhibitor and metformin is a biguanide; together they provide complementary mechanisms of action.

Direction
Adults: 1 tablet (Sitagliptin 50 mg/Metformin 1000 mg) twice daily with meals to reduce gastrointestinal side effects. Dose is individualised by a physician based on current therapy and renal function.
Side effects
- Nausea
- diarrhea
- abdominal pain (metformin)
- Nasopharyngitis
- upper respiratory tract infection (sitagliptin)
- Headache
- Hypoglycaemia (especially when combined with sulfonylureas or insulin)
- Lactic acidosis (rare but serious — metformin)
- Pancreatitis (rare)
Precautions
- Contraindicated in patients with eGFR < 30 mL/min/1.73 m² (severe renal impairment).
- Withhold before and 48 hours after iodinated contrast media administration.
- Risk of lactic acidosis from metformin component in renal
- hepatic impairment
- or with excessive alcohol.
- May cause pancreatitis
- discontinue if severe abdominal pain occurs.
- Not for use in type 1 diabetes or diabetic ketoacidosis.
- Use with caution in the elderly
- monitor renal function regularly.
- Keep out of reach of children.
