Indication
Medicime 200 (Cimetidine 200 mg/2 mL injection) is an H2-receptor antagonist used to treat peptic ulcer disease, Zollinger-Ellison syndrome, gastroesophageal reflux, and to prevent stress ulcers. Given by injection for acute conditions or when oral administration is not possible.

Direction
Adults: Administered by slow IV injection over at least 5 minutes (200 mg diluted in 20 mL of compatible solution), or by IM injection, every 4–6 hours. Typical dose: 200–400 mg IV/IM every 4–6 hours. Maximum: 2.4 g/day. Dosage adjusted for renal impairment.
Side effects
- Headache
- dizziness
- Diarrhea
- nausea
- Gynecomastia and sexual dysfunction with prolonged use
- Confusion
- particularly in elderly or renally impaired patients
- Elevated serum creatinine
- Drug interactions (CYP450 inhibition)
Precautions
- Inject IV slowly to avoid cardiac arrhythmias with rapid injection.
- Use with caution in patients with renal or hepatic impairment (dose reduction required).
- Cimetidine is a potent inhibitor of CYP450 enzymes — significant drug interactions with warfarin
- phenytoin
- theophylline
- lidocaine
- and many other drugs.
- May increase serum creatinine (without affecting true glomerular filtration).
- Use with caution in elderly patients (increased risk of confusion).
- Not recommended during pregnancy unless clearly necessary.
- Keep out of reach of children.
